Attention All Alcohol License  Holders  And Specifically Those  License Holders Under Section Twelve:

Application for renewal for 2012 are being accepted now until November 30th 2011.

Applications will be closed on November 30th 2011.  There will be no exceptions.

Please come early and make your application and bring with you a check, money order for the correct amount together with a copy of your menu if applicable.  No cash

Applications not completed will not be accepted.

Liquor liability insurance must be in effect in accordance with chapter 116 of acts 2010, together with certificate required under chapter 304 acts of 2004.

ADVISORY: LIQUOR LIABILITY INSURANCE FORMS

It is the responsibility of the License Holder to be certain the Liquor Liability Insurance Form (Accord 25) is current and in good standing when you apply for the renewal for 2011.   If this Form is not up to date the alcohol application will not be renewed.
